A $100.00 deposit is required to hold your appointment.
This can be paid by Square, through the button below


*Medical Form*

Medical Forms must be received a minimum of 10 days prior to your appointment , if not your $100.00 deposit is non refundable. 
Email Medical Forms to: permanentmakupbyjoyce@gmail.com do not send through text messaging or messenger. 


Due to the protocols that must be followed for Permanent Makeup there is a one week cancellation policy. If you cancel your appointment less than a week from your scheduled   appointment your $100.00 deposit is non refundable.

remaining service balance.

 Balance may be paid by Venmo, Cash or Check.
