Deposit
A $150.00 deposit is required to hold your appointment.
This can be paid by Square, through the button below
*Medical Form*
Medical Forms must be received a minimum of 10 days prior to your appointment , if not your $150.00 deposit is non refundable.
Medical forms will be sent to your email address through google docs and can be returned to permanentmakeupbyjoyce@gmail.com
Cancellation
Due to the protocols that must be followed for Permanent Makeup there is a one week cancellation policy. If you cancel your appointment less than a week from your scheduled appointment your $150.00 deposit is non refundable.
Payment Day of Service
To keep costs of services down
Square is not accepted for the
remaining service balance.
Balance may be paid by Cash or Check.
